All youth (6th - 12th grade) need to complete and return a 2012-2013 Health & Release Form before participating in any SPC youth activities. This ensures that the appropriate adults can be reached in case of an emergency and proper treatment can be given if something should happen. Each year, families must fill out a new form for each youth participating in SPC youth activities.
